FOR PET OWNERS
New client consultations are initially scheduled for 20 minutes up to a maximum of 60 minutes per client per session. You can always schedule additional time to discuss your pet’s condition, however, the initial time limit is 60 minutes.

How it works
STEP 1
Please complete the online registration form and the Pet Personality Form 3 business days PRIOR to your consultation. Please email the Pet Personality Form to drsignebeebe@gmail.com. This information is critical to accurately evaluate your pet’s condition. Please include all diets fed to your pet in the last 6 months. Please include the brand of food, any and all supplemental foods as well as treats given.
STEP 2
Medications/Diet/Supplements Section Instructions
Please include all drugs and over the counter medications given to your pet when you complete the form. This includes herbal medicines, homeopathics, joint care supplements and vitamins/mineral supplements. For all conventional drugs, over the counter medications/herbs/supplements, please include the dose given, how many times a day it is being given and for how long you have given your pets these medications or products. This information is critical for evaluation of your pet's condition
STEP 3
Please email all of your pet's medical and surgical records to drsignebeebe@gmail.com at least 3 business days prior to your consultation. This includes: all of regular veterinarian records (RDVM), laboratory reports and other diagnostic reports (radiographs, ultrasound, endoscopy, biopsy). Failure to provide previous veterinary records will slow and/or prevent a complete and comprehensive evaluation of your pet's condition at the time of initial consultation.
STEP 4
The amount of information given at the initial visit can be enormous. It is encouraged that you write down any information you want to keep. Likewise, if you have specific questions, please write them down before your consultation so they can be discussed during your appointment.
STEP 5
Be ready and on time for our appointment! If you need to cancel, contact me 48 hours in advance. If you have not completely filled out your paperwork or provided your pet’s records before your appointment, you will need to reschedule.
